How to ask for help
Application
Complete the entire four-page application. Incomplete or last-minute requests may not be processed by your deadline — submit within 30 days of your need.

Sign the Authorization to Use or Disclose Client Information form and send it with your application. Both documents are required.

Mail or email the signed forms to HOPE Fund of Penns Valley, P.O. Box 427, Centre Hall, PA 16828. Outreach will meet with you, then the board reviews requests monthly.
